Self-Assessment for Sit Down Service at our Rosebank cafe

Hello! You've found your way here because you're sitting at (or maybe waiting for) a table in our Rosebank cafe. We're so damn glad we get to host you in our cafe again, however, to help prevent the spread of the coronavirus and protect both you and our staff, we need to ask you to fill in this form before we can serve you.

All info is submitted via Google Forms and we'll hang on to it as long as regulations require. If you'd prefer not to submit any of this info for any reason we unfortunately are not permitted to offer you a seat, but you're more than welcome to grab a takeaway.

Please remember that space in our cafe is limited at the best of times, and even more so now. So, please be patient and help us out by only taking a seat if you intend to eat or drink a while.
